Community First Choice (CFC) is Connecticut's Medicaid-funded personal care and support program for people who need an institutional level of care — any age, no waitlist, delivered in the home of their choice.

For parents, caretakers, children, and pregnant women who meet income guidelines. Most common Medicaid plan for families.
For adults who are elderly, blind, or have a disability and meet income and asset requirements set by DSS.
For adults aged 19–64 who do not qualify for other Medicaid categories but meet the income threshold under the ACA expansion.

Cottage Homecare Services CT LLC is a licensed Medicaid provider for Adult Family Living (AFL) and related Connecticut programs. Families in all 8 counties can hire the caregiver they trust most, a parent, a sibling, a close family friend, and get paid to give the care they're already providing.

What's Covered
CFC covers a broad range of personal care and support services designed to help you maintain daily life safely at home.
Assistance with bathing, grooming, oral hygiene, skin care, and personal appearance as part of daily routines.
Help with selecting appropriate clothing and assistance with putting on or removing clothing and footwear.
Assistance with meal preparation, serving food, and helping with eating or drinking safely.
Help with moving around the home, getting in and out of bed, chairs, or wheelchairs safely.
Assistance with toilet use, incontinence care, and maintaining hygiene and dignity throughout.
Reminders and assistance with medication routines (non-clinical), supporting adherence to prescribed schedules.
Assistance with essential household tasks — laundry, dishes, vacuuming — to maintain a safe, clean living environment.
Help accompanying individuals to appointments, grocery shopping, and community activities when needed.
Our registered nurses coordinate your care plan, monitor quality, and communicate with DSS and healthcare providers on your behalf.
Service hours and scope are determined by your individual DSS assessment — our team helps document your needs accurately.

How It Works
Three simple steps from your first call to receiving care at home.
Call us or fill out the form to begin. Our team will review your Medicaid status and functional needs to confirm your eligibility for CFC at no cost.
Our registered nurse conducts a home assessment and documents your care needs for DSS. We guide you through all required paperwork to help support a smooth approval process.
Once approved, your chosen caregiver is enrolled and trained. Care begins in your home on a schedule that works for you — paid through the Medicaid program.
